To register your child, grandchild, or other child who qualifies, please fill out all sections of the form below.  There is no cost involved, the child will receive books free each month.  The only requirement is that the child (not the parents or grandparents) live within Polk County, Texas, and be under the age of five.  Please be sure to include parent's email address or the registration will not be accepted.
